Healthcare Job Details

• Location: Louisville, KY – Kentucky healthcare jobs in a city known for its culture, history, and welcoming community.
• Employment Type: Locum tenens (contract healthcare position)
• Schedule: Monday to Friday, 8:00am – 5:00pm
• Shifts per Week: 5
• Shift Duration: 8-hour days
• Hours per Week: 40
• Patient Load: Maximum 6 patients per day
• No on-call required
• Contract Duration: 13 weeks, with potential for extension
• Start Date: August 25, 2025
• Estimated Weekly Salary Range: $4,110 - $4,870 per week

Nurse Practitioner / Physician Assistant Job Requirements

• Minimum 1 year of clinical experience with aging adults or adults with complex care needs
• Nurse Practitioner: Graduation from an accredited health professions school; ANCC or AANP certification (PMHNP or NNP not eligible)
• Physician Assistant: Board certification required
• Active Kentucky state NP or PA licensure
• Active DEA license and Kentucky controlled substance registration (as applicable)
• Basic Life Support (BLS) certification
• Experience in home-based healthcare or congregate living settings preferred
• Telehealth and virtual care experience preferred
• Proficient computer and software skills
• Ability to travel locally; valid driver’s license, clean driving record, and insurance
• Advanced training in geriatrics or palliative medicine preferred
• Experience with managed care or value-based healthcare delivery preferred

Healthcare Responsibilities

• Provide individualized patient care, including urgent and chronic condition management
• Collaborate with nurses, medical assistants, social workers, pharmacists, and other healthcare professionals
• Deliver clinical care in private homes, nursing facilities, ambulatory clinics, and virtually
• Utilize clinical data dashboards to support healthcare performance goals
• Promote patient engagement and experience initiatives
• Maintain accurate clinical documentation and coding
• Participate in care team huddles and operational meetings
• Coordinate care with internal and external healthcare team members
• Follow medical policies, protocols, and clinical guidelines
• Build relationships with patients, caregivers, facility staff, and clinical partners
• Perform other market-specific healthcare duties as assigned
