Key Job Details – Oregon Nurse Practitioner Opportunity

• Job Title: Locum Tenens Field-Based Nurse Practitioner – Homecare
• Location: Josephine County, Oregon (Oregon healthcare jobs, southern Oregon nursing opportunities)
• Position Type: Locum tenens (1099/PRN)
• Setting: Field-based, in-home patient visits
• Shift: 8-hour days, 5 shifts per week (40 hours/week)
• Duration: 26 weeks, with potential for extension
• Start Date: August 25, 2025
• End Date: February 21, 2026
• Minimum Commitment: 30 hours per month
• Estimated Hourly Pay: $100 per visit 

Nurse Practitioner Job Requirements

• Active, unencumbered Nurse Practitioner license in Oregon
• Minimum 1 year of clinical patient care experience required (3+ years preferred, especially in primary care, adult/geriatric, or homecare settings)
• Experience with in-home risk assessments preferred
• Ability to travel within Josephine County (up to 55-mile radius); reliable transportation required
• Strong EMR documentation and clinical assessment skills
• Ability to work independently and communicate effectively with patients
• Bilingual skills are a plus

Nurse Practitioner Responsibilities

• Conduct comprehensive annual in-home wellness visits and risk adjustment health assessments for Medicare and at-risk populations
• Perform physical exams, review body systems, vital signs, medication history, and current prescriptions
• Assess member functional status, pain management needs, cognitive function, home environment, fall risk, and social determinants of health
• Provide patient education on health topics such as pain management and medication adherence
• Build trust and rapport with members through empathetic communication
• Assist in closing quality care gaps (e.g., screenings and labs)
• Document all assessments and findings accurately in the EMR system
• Collaborate with healthcare teams to enhance care delivery and patient outcomes
