• Job Title: Locum Tenens Field-Based Nurse Practitioner – Homecare
• Location: Coos County, Oregon (Oregon nurse practitioner jobs, coastal healthcare opportunities)
• Position Type: Locum tenens (1099/PRN)
• Setting: Field-based, in-home patient visits
• Schedule: 5 shifts per week, 8-hour days
• Hours: 40 hours per week; minimum 30 hours per month
• Assignment Duration: 26 weeks, with potential for extension
• Start Date: August 25, 2025
• End Date: February 21, 2026
• No on-call required; flexible scheduling

Nurse Practitioner Qualifications

• Active, unencumbered Nurse Practitioner license in Oregon
• Minimum 1 year of clinical patient care experience (3 years preferred in primary care, adult, or geriatric healthcare settings)
• Proficiency with EMR systems
• Ability to travel within a 55-mile radius of Coos County (reliable transportation required)
• Self-motivated and able to manage an independent schedule
• Prior in-home risk assessment or locum tenens/PRN experience preferred
• Bilingual skills are a plus

Primary Responsibilities – Homecare Nurse Practitioner

• Conduct annual in-home wellness visits and risk adjustment health assessments for Medicare and at-risk members
• Perform comprehensive physical exams, medication reviews, functional and cognitive screenings, and home environment evaluations
• Provide patient education on health topics such as pain management and medication adherence
• Build trust and rapport with patients through excellent communication and empathy
• Assist in closing care gaps, including preventive screenings and laboratory tests
• Document clinical findings accurately and communicate with interdisciplinary care teams
• Collaborate to improve care delivery and patient outcomes in the community
