Key Job Details – Nurse Practitioner (Homecare, Locum Tenens)

• Location: Clackamas, Oregon (field-based, travel within a 55-mile radius of Clackamas County)
• Employment Type: Locum tenens (1099/PRN) – perfect for Nurse Practitioners seeking flexible healthcare jobs
• Schedule: 5 shifts per week, 8-hour days
• Hours: 40 hours per week (minimum 30 hours/month commitment)
• Assignment Duration: 26 weeks, with immediate start available
• Flexible work schedule – no on-call shifts required
• Paid mileage and dedicated coordinator support included
• Estimated Weekly Pay: $4110 - $4870

Nurse Practitioner Qualifications

• Active, unencumbered Nurse Practitioner license in Oregon
• Minimum 1 year of clinical patient care experience (3 years preferred, especially in primary care, adult, or geriatric healthcare settings)
• Experience with in-home risk assessments preferred
• Prior 1099 (PRN) healthcare job experience is a plus
• Ability to travel within Clackamas County (up to 55-mile radius); reliable transportation required
• Strong EMR proficiency and ability to work independently
• Bilingual skills are a plus

Primary Responsibilities – Field-Based Nurse Practitioner

• Conduct annual in-home wellness visits and risk adjustment health assessments for Medicare and at-risk members
• Complete comprehensive physical exams, medication reviews, cognitive and mental health screenings, and evaluate home environment and functional status
• Provide patient health education on pain management, medication adherence, and preventive care
• Build trusting relationships with patients through excellent communication and empathy
• Assist in closing quality care gaps, including screenings and labs
• Collaborate with healthcare teams to enhance care delivery and patient outcomes
