A Bit About Us

We are a local PACE organization looking for a Nurse Practitioner to join our team! As a member of our care team, you will work alongside MDs, NPs, Physical Therapists, Registered Dietitians, CNAs, and our office staff.

Some of the daily duties of the NP will include

• Treat patients with episodic illnesses
• Perform history and physicals with new patients
• Refer patients to specialists as needed
• Coordinate 24 hour care delivery
• Provide occasional after hour call
• Document all notes in our EMR system
• Work with the care team to develop treatment plans
• Provide world class care on a 1 1 level with the patients

We are currently seeking a highly skilled, dedicated, and compassionate Permanent Nurse Practitioner with a wealth of experience or interest in PACE (Program of All-Inclusive Care for the Elderly) to join our dynamic healthcare team. The chosen candidate will play a pivotal role in providing comprehensive, coordinated care to seniors with health and functional challenges. This is an exceptional opportunity for a Nurse Practitioner looking to make a profound impact on the lives of our patients, delivering high-quality care that promotes their independence, dignity, and choice.

Responsibilities

• Provide comprehensive healthcare services to PACE participants, including diagnosis, treatment, and consultation.
• Collaborate with interdisciplinary teams to develop, implement, and evaluate individualized care plans for patients.
• Conduct comprehensive assessments of physical, emotional, psychosocial, and functional status of patients, and provide appropriate interventions.
• Monitor and manage patients’ response to treatment plans, modifying as necessary.
• Advocate for patients and their caregivers, providing education and support to ensure understanding of treatment plans and adherence to prescribed care.
• Participate in interdisciplinary team meetings, contributing professional nursing knowledge and skills in the development and implementation of individualized care plans.
• Maintain accurate, detailed reports and records, ensuring compliance with regulatory standards and organizational policies.
• Participate in quality improvement initiatives and educational programs to stay updated on the latest developments in geriatric nursing and PACE.
  
  

Qualifications

• Master’s Degree in Nursing from an accredited institution.
• Current, unrestricted Nurse Practitioner license in the state of practice.
• Certified as an Adult Geriatric Nurse Practitioner.
• Minimum of 2 years of clinical experience as a Nurse Practitioner, preferably in a PACE or geriatric setting.
• Demonstrated knowledge of PACE regulatory requirements and principles of geriatric medicine.
• Excellent diagnostic, clinical, and interpersonal skills.
• Exceptional communication skills, with the ability to interact effectively with patients, families, and healthcare team members.
• Strong commitment to improving the health and well-being of seniors.
• Ability to work independently, as well as part of an interdisciplinary team.
• Proficiency in electronic medical records and healthcare IT systems.
