Employment Type: Contract

Compensation: 100/hr

Schedule:

• Flexible on full or part time schedules
• Open to days, evenings or nights
• Must commit to a minimum of 30 assessments/month

Position Overview:

Our client is seeking Nurse Practitioners to support a new Medicaid home care contract. The primary responsibility is to conduct initial health assessments for members in their homes. Flexibility is available for both full-time and part-time positions.

Responsibilities:

• Conduct comprehensive histories and physical exams, evaluate social determinants, and deliver health education
• Order diagnostics, develop and adjust treatment plans, and manage medications (as applicable)
• Educate on disease prevention, medication use, lifestyle changes, and self-care
• Complete and submit visit documentation within 24 hours (can be completed during the visit).

Qualifications:

• Nurse Practitioner or Physician Assistant License in the stay of New York
• Minimum 1 year of NP/PA experience
• Ability to commit to 30 assessments within 1 month
• Previous home care experience preferred
