Position Overview
SCOPE OF ROLE:
The ACT teams Psychiatric Nurse Practitioner partners with the multi-disciplinary team to deliver comprehensive mental health treatment to individuals who have a mental illness that seriously impairs their functioning in the community. Serving those whose needs have not been met by traditional clinic-based approaches, this innovative, recovery-oriented, ‘best practice’ intervention delivers treatment to people in their homes, shelters, parks and anywhere else they happen to be living, engaging them in a sustained relationship that leads to recovery. ACT Teams are a mobile interdisciplinary team of professionals (psychiatry, nursing, social work, rehabilitation).
Must be registered as a NY State Medicaid provider. Must have a DEA# and an individual NPI #.

ESSENTIAL DUTIES & RESPONSIBILITIES:
- Write prescriptions for psychotropic medications as allowed by state regulations and collaborative practice agreements.
- Review health assessments completed by the Program Nurse and provide clinical feedback.
- Provide medication management including prescription refills and medication education to consumers.
- Document patients' medical and psychological histories, physical assessment results, diagnoses, treatment plans, prescriptions, or outcomes.
- Provide training to other team members on topics including medications and drug interactions.
- Communicate and coordinate care with psychiatrists in the community, as needed.
- Assist with psychiatric emergencies, refer residents to emergency care if needed.
- Provide crisis intervention and address all emergency situations as appropriate.
- Maintain up to date, accurate, complete, and timely consumer chart documentation.
- Attend staff meetings, case conferences, group supervision, and trainings as needed.
- Comply with all SUS Employee Policies, Procedures, and Protocols, SUS Corporate Compliance Policies, and NYS Dept. of Social Services Rules, Regulations, and Statutes.
- Coordinates care with S:US nurse practitioners, medical director and administrative staff; closely coordinates with clinicians external to S:US as appropriate.
- Utilizes best and evidence-based approaches to treatment consistent with S:US' organizational culture; health/mental health integration, rehabilitation and recovery, and wellness approaches figure prominently.
- Perform other related duties as assigned.

Qualifications
REQUIRED EDUCATION AND EXPERIENCE
- Licensed as a Registered Nurse in New York State and credentialed by the State to practice as an advanced Nurse Practitioner with a specialty in psychiatry. Must be registered as a NY State Medicaid provider. Must have a DEA# and an individual NPI #.
- Maintain CPR/First Aid Certification.
- Preference is given to those who have experience working on a multi- disciplinary team and/or previous experience with severely and persistently mentally ill, substance use and homeless populations. Ability to work independently and as part of a team
