Responsibilities

• Provide comprehensive in-home and community-based healthcare services, including medical history reviews, physical examinations, and medication management
• Formulate diagnoses and develop individualized treatment plans
• Maintain accurate and up-to-date electronic medical records within 48 hours of visits

Qualifications

• active FNP, AGNP, or PA license
• active DEA license
• current BLS Certification
• valid driver’s license with a minimum of 3 years with a clear driving record
• valid proof of car insurance
