About The Role
The Advanced Practice Provider- Medical Management (APP) role is a hybrid position. This role will primarily be a remote position leveraging telehealth/virtual visits with in-person visits occurring when clinically appropriate and at the discretion of clinical leadership. This position focuses on medical oversight and management of a designated Longevity Health member panel in partnership with the Longevity Health RN Field Coordinator and other ancillary providers. The APP conducts assessments for chronic disease management, acute changes in condition, and transitions of care. Frequent collaboration and communication with the primary team, the members and their family/loved ones are instrumental in coordinating care that aligns to the member’s goals of care and needs. As part of the Clinical team, the APP will be supported by the Clinical Manager, Market Director of Clinical Services, and local medical director who provide education and guidance on best practices to effectively care for their members.

Key Tasks And Responsibilities
- Provides telehealth/virtual and/or in-person (as needed) collaborative primary care and services
- Participates in comprehensive documentation and quality initiatives.
- Supports treating the member in the most appropriate setting and executes on the treat in place model
- Responds to documentation queries.
- Collaborates and communicates with family members/responsible parties and the ICT team.
- Promotes Longevity Health with members, loved ones and nursing facilities and other stakeholders.
- Functions autonomously.
- Is member-centric and customer service oriented, establishing meaningful relationships that ensure effective and high-quality care for Longevity Health members.
- Effectively prioritizes work.
- Sets goals, develops, and executes effective treatment plans.
- Understands conditions common to the long-term nursing home population and uses evidenced-based guidelines and experience to make effective decisions.
- Identifies and responds to potential patient changes in condition.
- Documents member care in the Electronic Medical Records timely and accurately.
- Facilitate members’ access to benefits and services.
- Supports the review of high acuity events such as hospitalizations as directed by the manager.
- Understands health plan benefits and services.
- Understands the nursing home regulatory environment.
- Effectively communicates verbally and in writing.
- Resolves conflicts/overcomes barriers in an effective and appropriate manner.
- Responsible for maintaining licenses, board certifications, DEA, and annual requirements.
- Adheres to all Longevity Health Policies/Procedures/Guidelines.
- Maintains compliance with Medicare, HIPAA, PHI and Cyber Security rules, laws.
- Attends meetings and educational sessions.
- Displays respectful, collaborative team approach to colleagues.
- Performs other related duties as assigned.
- Active, State unrestricted licensed Advanced Practice Nurse, National Board certification (AANP, ANCC or AACN) as an Adult, Family, Geriatric or Primary Care nurse practitioner required. Will allow acute care nurse practitioner with nursing home/geriatric or hospice palliative experience OR state unrestricted licensed Physician’s Assistant NCCPA board certified with nursing home/geriatric or hospice/palliative experience.
- Active DEA License registered in the state of practice is required.
- Must have an active national provider number (NPI) and an active Medicare provider number.
- Must be eligible to participate in the Medicare program as a provider and registered.
- Valid state driver's license with a good driving record and proof of automobile insurance required.
- Auto insurance coverage as required by your state of residence. New employees will be responsible for providing proof of coverage prior to hiring and on an ongoing basis. Must be willing to travel within a market, if deployed by leadership.
- Master of Science in Nursing as a Nurse Practitioner from a nationally accredited program OR Master’s Degree from a nationally accredited program.
- Master of Physician Assistant Studies or Master of Clinical Health Services from an accredited college or university for Physician Assistance candidates required.
- Minimum of 4 years experience as a Nurse Practitioner/Physician’s Assistant AND has some experience with geriatric/hospice or palliative care population prior to becoming an NP/PA.
- Experience working in a nursing home environment is strongly desired.
- Computer/Technology proficiency required; telehealth experience preferred.
- Excellent clinical skills—assessment, diagnosis, treatment, prescribing specifically for the chronically ill population.
- Bilingual abilities are beneficial but not required.
