Position Description
Title: Nurse Practitioner
Location: New York City, NY (Hybrid)
Employment Type: Temp to Perm, Day Shift, Mon-Fri 08:00am-05:00pm (3 Days/week)
Compensation
Pay Range: $63.70-$91.00 Per Hr
Description
Collaborate with community primary care physicians to oversee and coordinate participant care.
Support the participant's functional, clinical, and psychosocial needs, focusing on independence and wellness.
Act as a liaison between the participant, community physician, and plan staff to develop and implement care plans.
Conduct annual wellness assessments and address higher risk participants more frequently.
Provide urgent care services and facilitate same-day access.
Participate in clinical documentation improvement activities.
Educate participants and their families on disease processes, medication adherence, and self-management.
Key Responsibilities
Supervise and manage Nurse Practitioners and other clinical staff as needed.
Complete direct reports and annual performance appraisals.
Perform wellness assessments and manage urgent care services.
Reconcile medications and treatment plans post-hospitalization.
Review past medical history and document diagnoses accurately.
Co-manage high-risk participants with primary care physicians.
Conduct home visits and supervise home care services as needed.
Qualifications
Graduated from a nurse practitioner education program acceptable to NYSED or certified by a national certifying organization.
Minimum of two years in clinical nursing practice in-home care, care management, or related field.
Minimum of one year of experience working with a frail or elderly population.
Licensed to practice as a Nurse Practitioner and Registered Nurse in NY.
DEA Certification and AANP or ANCC Board Certification required.
Effective oral and written communication and interpersonal skills.
Basic computer skills in Microsoft Office.
