Position Description
Columbia Memorial Hospital (CMH) is seeking an experienced Nurse Practitioner to join the CMH Medical Group as part of our CMH-OHSU Health Primary Care Clinic in Warrenton on the Oregon north coast.
We invite you to join our Medical Group where you will practice medicine in a rural outpatient Primary Care setting. Our clinics offer a supportive work environment where you will be able to provide full spectrum care for patients of all ages alongside primary care physicians and advance practice practitioners.
The Primary Care Provider Extender position will not manage a panel of their own but rather serve as an extension of the existing Primary Care Providers. This individual would be responsible for seeing follow-up visits, urgent visits, TCM visits, pre-operative clearance visits to decompress existing provider schedules and improve access to care. They would also be responsible for assisting with provider inbox messages (patient messages, medication management, diagnostic results and orders, and responding to urgent issues) to ensure patients receive timely care.

Knowledge Of/Skill/Ability To
- Strong oral and written and interpersonal communication skills.
- Organizational and problem solving skills.
- Must be able to handle multiple tasks and exercise sound clinical and independent judgments in all age groups served.
- Demonstrated experience in area of clinic practice is required.
- Excellent written verbal, computer and communication skills are required.
- Completion of an accredited Nurse Practitioner training program.
- Preferably two or more years of experience.
- Current Nurse Practitioner license for the State of Oregon at the time of hire.
- State of Oregon Nurse Practitioner Prescriptive privileges.
- ACLS or BLS required through the American Heart Association.
- Must be successfully credentialed by the CMH Professional Staff.
