About The Job

The Senior Community Care (SCC) program is a longitudinal, integrated care delivery initiative focused on coordinating clinical care for high-risk health plan members in facility settings. As an Advanced Practice Clinician (APC), you’ll be part of an interdisciplinary team delivering personalized, innovative care. Our client offers flexible scheduling with no on-call, weekend, or holiday requirements—supporting a healthy work-life balance.

Responsibilities

• Conduct comprehensive assessments
• Manage acute and chronic medical and behavioral conditions
• Ensure accurate documentation, including ICD-10 coding
• Collaborate with interdisciplinary care teams
• Lead advanced illness and care planning conversations
• Educate and counsel patients and caregivers
  
  

✅ Minimum Qualifications

• Active and unrestricted license in practicing state (or ability to obtain by start date)
• Education:
• NP: Master of Science in Nursing or Doctor of Nursing Practice from an accredited program
• PA: Degree from an accredited Physician Assistant program
• Certification:
• NP: Board certified by AANP or ANCC in one of the following:
• Family Nurse Practitioner
• Adult Nurse Practitioner
• Gerontology Nurse Practitioner
• Adult-Gerontology Acute Care Nurse Practitioner
• PA: Certified by the National Commission on Certification of Physician Assistants
• Active DEA licensure/prescriptive authority
• Physical ability to lift 30-pound bag, navigate stairs, and perform assessments
• Reliable transportation
• Ability to obtain collaborative practice agreement (if applicable)
• Full COVID-19 vaccination (required by UnitedHealth Group policy)
• Must pass pre-employment drug screening
  

⭐ Preferred Qualifications

• 1+ year of clinical experience (long-term care preferred)
• Experience with patients with complex behavioral, social, or functional needs
• Knowledge of geriatrics and chronic illness management
• Familiarity with advanced illness and end-of-life care discussions
• Proficient in electronic and written medical documentation
