Schedule: Full-time, 40 hours/week | Monday–Friday, 8:00 AM–5:00 PM

Duration: Approximately 6 months (FMLA coverage)

Travel: Local travel during shift hours; single-facility assignment

About The Role

Join our client's Senior Community Care (SCC) program—a longitudinal, integrated care model designed to support high-risk health plan members in facility-based settings. As an Advanced Practice Clinician (Nurse Practitioner or Physician Assistant), you’ll be part of an interdisciplinary team delivering personalized, proactive care to seniors in a skilled nursing environment.

️ Key Responsibilities

• Perform comprehensive clinical assessments
• Manage acute and chronic medical and behavioral health conditions
• Document care accurately using ICD-10 coding standards
• Collaborate with interdisciplinary care teams
• Lead advanced illness and care planning discussions
• Educate and counsel patients and caregivers
  
  

✅ Basic Qualifications

• Active and unrestricted NP or PA license in the state of practice (or ability to obtain by start date)
• NP: Master of Science in Nursing or Doctor of Nursing Practice from an accredited program
• PA: Graduate of an accredited Physician Assistant program
• Board certification:
• NP: AANP or ANCC in one of the following specialties:
• Family Nurse Practitioner
• Adult Nurse Practitioner
• Gerontology Nurse Practitioner
• Adult-Gerontology Acute Care Nurse Practitioner
• PA: NCCPA certification
• Active DEA license and prescriptive authority
• Ability to meet physical demands (e.g., lifting 30 lbs, navigating stairs, performing physical assessments)
• Reliable transportation
• Ability to obtain collaborative practice agreement (if required by state)
• Fully vaccinated against COVID-19 (compliant with federal, state, and client requirements)
• Must pass pre-employment drug screening
  

Preferred Qualifications

• 1+ year of clinical experience (long-term care preferred)
• Experience managing patients with complex behavioral, social, or functional needs
• Familiarity with geriatrics, chronic illness, and end-of-life care
• Proficiency in electronic medical records and clinical documentation
