Program: ISNP – Senior Community Care (SCC)

Type: Potential Temp-to-Hire

Schedule: 40 hours/week | Monday–Friday | 8 AM–5 PM

Role Overview

Join a longitudinal, integrated care program serving high-risk members in facility settings. As part of an interdisciplinary team, you'll deliver personalized care with flexible scheduling—no on-call, weekends, or holidays.

Primary Responsibilities

• Conduct comprehensive assessments
• Manage acute/chronic medical & behavioral conditions
• Document accurately (ICD-10)
• Collaborate with interdisciplinary care teams
• Lead advanced illness & care planning conversations
• Educate patients and caregivers
  
  

Required Qualifications

• Active/unrestricted NP or PA license (or ability to obtain)
• NP: MSN or DNP from accredited program
• PA: Accredited PA degree
• Board certification:
• NP: AANP or ANCC (FNP, ANP, GNP, AGACNP)
• PA: NCCPA
• DEA license/prescriptive authority
• Physical ability: lift 30 lbs, navigate stairs, perform assessments
• Reliable transportation
• Collaborative practice agreement (if applicable)
• Fully vaccinated for COVID-19
  

✅ Preferred Qualifications

• 1+ year clinical experience (long-term care preferred)
• Experience with complex behavioral/social/functional needs
• Knowledge of geriatrics, chronic illness, end-of-life care
• Proficient in EMR documentation
