Qualifications for Advanced Practice Providers

• Minimum 1 year of clinical experience as a Nurse Practitioner or Physician Assistant
• Graduate of an accredited health professions program with active national board certification
• Active California Nurse Practitioner or Physician Assistant license
• Current DEA registration and state-specific controlled substance license (if applicable)
• BLS (Basic Life Support) certification
• Valid driver's license, proof of insurance, and ability to travel to patient homes in San Diego
• Minimum 5 years licensed with a clean driving record (no suspensions, DUIs, or felonies in the past 7 years)
• Fully vaccinated (medical or religious exemptions considered with documentation)

Primary Care Responsibilities

• Provide comprehensive in-home primary care, urgent care, and chronic disease management to adult patients
• Develop strong patient and caregiver relationships, focusing on patient-centered care and wellness
• Collaborate with a multidisciplinary healthcare team to coordinate care transitions and address social determinants of health
• Participate in after-hours on-call coverage as needed
• Engage in ongoing professional development and clinical education
