Hybrid Nurse Practitioner – Full Time (Salinas / Soledad, CA)

We are seeking a Full-Time Hybrid Nurse Practitioner to join our dedicated clinical care team serving patients in the Salinas and Soledad, CA region. This is an excellent opportunity for an experienced Nurse Practitioner who is passionate about providing patient-centered care and supporting individuals with complex health needs.

About the Role

• Schedule: Monday–Friday, full time, self-managed

• Hybrid model: 3–4 days per week in clinic, 1–2 days remote for administrative/telehealth work

• Location: Open to candidates who reside in Monterey County, and are able to travel to Salinas and Soledad clinics at least 2 days per week

As a Hybrid Nurse Practitioner, you will:

• Provide direct patient care, education, and care coordination for individuals with chronic and complex medical needs

• Develop tailored care plans in collaboration with patients, families, and the care team

• Work with an integrated care team to improve care consistency, enhance patient services, and support overall health outcomes

• Spend ~70–75% of your time in clinic, with ~25–30% of care delivered via telehealth

• Manage a small panel of patients (approx. 10 per week) with extended visit times (3.5–4 hours each)

Requirements

• Current Nurse Practitioner license in California

• DEA license (or eligible within 90 days of hire)

• BLS/CPR certification

• Valid driver’s license, insurance, and reliable transportation

• Minimum 2 years of experience as a Nurse Practitioner (Family, Internal Medicine, Hospitalist, Cardiology, or Nephrology experience preferred)

• Strong clinical assessment and care planning skills

• Proficiency with Cerner EMR (or similar) and Microsoft Office Suite
