Job Title: Nurse Practitioner / Physician Assistant – Fee-Based Initial Health Assessment (IHA)
Location: Cobb County, GA (Must be willing to travel within a 50-minute radius of home)
Job Type: Contract / Fee-Based (No guaranteed hours; ~30 hours/month)
Reports to: Manager of Care Delivery
FLSA Status: Exempt
About The Role
As a Fee-Based Nurse Practitioner or Physician Assistant, you’ll conduct comprehensive Initial Health Assessments (IHAs) for Medicare, Medicaid, and ACA members. These assessments are performed in-person at members' homes and occasionally via virtual visits. Your work will directly impact member health outcomes by identifying risks and supporting preventative care strategies.

About The Role
As a Fee-Based Nurse Practitioner or Physician Assistant, you’ll conduct comprehensive Initial Health Assessments (IHAs) for Medicare, Medicaid, and ACA members. These assessments are performed in-person at members' homes and occasionally via virtual visits. Your work will directly impact member health outcomes by identifying risks and supporting preventative care strategies.
Responsibilities
- Conduct thorough IHAs including medical history, physical exams, and screenings in members’ homes
- Deliver high-quality, empathetic care that meets health plan expectations and member satisfaction standards
- Accurately document findings in the EMR within 24 hours; respond to queries within 48 hours
- Maintain availability for a minimum of 30 appointments per month
- Collaborate with clinical leadership and maintain a physician agreement
- Ensure reliable transportation and punctuality for all scheduled visits
- Comply with all federal, state, and organizational policies, including HIPAA and ethical standards
- Participate in ongoing training and team meetings
- Master’s or Doctorate from an accredited NP (FNP, AGNP, ANP) or PA program
- Active RN and NP or PA license in the state of practice
- Board certification (ANCC, AANP, or NCCPA)
- Current Medicaid ID and Medicare PTAN
- BLS certification
- Valid driver’s license and proof of auto insurance
- Ability to pass comprehensive background and drug screening
- Willingness to sign a contractual agreement outlining responsibilities and terms
- Strong clinical judgment and decision-making skills
- Proficiency with EMR systems and digital documentation
- Deep understanding of CMS and ACA risk adjustment models
- Excellent verbal and written communication
- Empathy, professionalism, and respect in member interactions
- Ability to work independently and adapt to changing conditions
- Commitment to confidentiality and HIPAA compliance
- Proactive problem-solving and collaboration with care teams
- Ability to lift up to 20 lbs and kneel/bend as needed during assessments
- Frequent travel to member homes; exposure to varied home environments
- Ability to remain stationary, communicate clearly, and perform repetitive motions
- Mental agility to interpret data, plan, and make decisions
- Experience in home health or in-home assessments
- 1+ year working with Medicare/Medicaid populations
- Fee-based per completed assessment (market rate)
- Mileage reimbursed at IRS rate after first 30 miles/day
