Job Title: Longitudinal Care Nurse Practitioner

Location: Orange & Lake County, FL

Responsibilities

• Provide comprehensive in-home and community-based healthcare services, including medical history reviews, physical examinations, and medication management.
• Conduct thorough patient assessments, formulate diagnoses, and develop individualized treatment plans.
• Manage chronic health conditions and promote preventive care measures.
• Prescribe and manage medications, educating patients on proper administration.
• Serve as the patient's primary care provider of record.
• Maintain accurate and up-to-date electronic medical records within 48 hours of visits.
• Exhibit exceptional bedside manner, fostering trust and rapport with patients.
• Collaborate with other healthcare professionals to ensure coordinated care.
• Participate in after-hours on-call rotations as needed.
• Travel to and from patient appointments in assigned territories, seeing a minimum of 5-6 patients per day.
  
  

Qualifications

• Active, unrestricted licensure as a Provider (Family Nurse Practitioner, Adult-Gerontology Nurse Practitioner, or Physician Assistant) in Arizona.
• Active Drug Enforcement Administration (DEA) license.
• Current Basic Life Support Certification for healthcare providers.
• Strong clinical assessment and diagnostic skills.
• Excellent communication and patient education abilities.
• Proficiency in electronic medical record (EMR) systems.
• Valid driver's license for a minimum of 3 years with a clear driving record (no DUIs or felony driving convictions in the last 7 years).
• Valid proof of sufficient car insurance coverage and access to reliable transportation.
• Ability to pass a comprehensive background check and drug screen.
• 1+ year of experience in primary care or a related field (family practice, general practice, or outpatient care) in the Arizona market.
