Job Title: Home Health Nurse Practitioner (NP)

Location: Phoenix, AZ

Schedule: Monday – Friday, 8:00 AM – 5:00 PM

Overview

We are seeking a compassionate and experienced Nurse Practitioner (NP) or Physician Assistant (PA) to join our growing home health team in Phoenix, AZ. This role focuses on providing high-quality, patient-centered care in the comfort of patients’ homes. You'll conduct comprehensive health assessments, develop personalized treatment plans, and educate patients on disease management and healthy living.

This is a flexible, field-based position with the potential for extra income and the opportunity to build long-term relationships with patients through longitudinal care.

Key Responsibilities

• Perform thorough patient histories and physical exams during home visits
• Evaluate and address social determinants of health impacting care and outcomes
• Order and interpret appropriate diagnostic tests
• Develop, implement, and adjust evidence-based treatment plans
• Prescribe and manage medications in accordance with scope of practice
• Deliver patient and caregiver education on disease prevention, lifestyle modifications, medications, and self-care
• Complete all documentation and submit visit notes within 24 hours (preferably during the visit)

Qualifications – Must Have

• Active, unrestricted NP or PA license in the state of Arizona
• Minimum of 1 year of clinical experience as an NP or PA
• Ability to independently manage a caseload and complete 30 assessments per month
• Strong interpersonal and communication skills
• Reliable transportation for home visits

Preferred Qualifications

• Prior experience in Home Health or field-based care
• Familiarity with EMR systems and mobile documentation tools
