Position Description

Job Title: Nurse Practitioner - Initial Health Assessment (IHA)

Locations:

  • Current Openings: Fulton County, Carroll County, Cobb County and Gwinnett County

Position Overview:

We are seeking compassionate and detail-oriented Advanced Care Providers to perform Initial Health Assessments (IHAs) for patients in their homes or via telehealth. This role plays a critical part in connecting patients to the care they need by providing comprehensive assessments that inform their ongoing healthcare.

This is a flexible opportunity designed to work around your schedule, with the majority of visits occurring in-home, along with some telehealth sessions.

Compensation:

  • $100 per In-Home Assessment (majority of visits)
  • $70 per Telehealth Assessment
  • $50 per No-Show
  • $550 One-Time Training Fee

Responsibilities:

  • Perform thorough and accurate Initial Health Assessments (IHAs) in-home or via telehealth
  • Review medical histories, current medications, and chronic condition management
  • Conduct basic physical exams and screenings as appropriate
  • Identify health gaps, risks, and preventive care opportunities
  • Document findings accurately and submit timely reports
  • Communicate respectfully and effectively with patients from diverse backgrounds
  • Collaborate with internal teams to coordinate follow-up care and referrals

Qualifications:

  • Active and unrestricted Nurse Practitioner (NP)
  • Valid DEA (if required for scope)
  • Experience performing health assessments preferred
  • Strong clinical documentation skills
  • Excellent bedside manner and interpersonal communication
  • Reliable transportation for in-home visits
