The assessments will include the following responsibilities:

- Reviewing patient's past medical history

- Review of systems

- Physical Exam

- Medication Review

- Depression Screening

- Checking Vitals

- Formulating list of current and past medical conditions

Qualifications

- Active NP license

- MUST be AANP or ANCC certified

- Minimum of 6 months of PAID NP experience

- Must be either Family (FNP), Adult-Gerontology (A-GNP), Adult (ANP-BC), or Adult-Gerontology Acute Care (AGACNP-BC) certified

- BLS/CPR Certified
