The Center for Hematologic Malignancies (CHM) outpatient clinic provides acute and long-term care to patients who have undergone an autologous or allogeneic hematopoietic cell transplant (HCT) or received other cellular therapies such as chimeric antigen receptor T-cell (CAR-T) therapy.

We are seeking a dedicated and experienced Infusion APP to join our Hematology & Medical Oncology team. This role is responsible for the clinical management of patients in the outpatient infusion unit, ensuring the delivery of coordinated, high-quality, and efficient care.

Location: South Waterfront - OHSU Center for Health & Healing, Building 2 (CHH2)

Schedule

  • Weekdays: 7:00 AM – 7:30 PM (7 days on / 7 days off)
  • Weekends: 8:00 AM – 6:30 PM
  • Coverage: Monday through Sunday
  • Note: Unit operates 365 days a year

Key Responsibilities

  • Provide comprehensive clinical management of patients receiving infusion therapy in an outpatient oncology setting.
  • Ensure a seamless and effective care experience through collaboration with an interdisciplinary team.
  • Partner with clinic leadership to identify and implement process improvement initiatives.
  • Serve as a resource for education and training of staff, colleagues, and students.

Function/Duties of Position

Clinical Responsibilities

  • Provide medical coverage for the Outpatient Treatment Unit
  • Respond to and manage adverse infusion reactions and other emergencies
  • Assess patient condition when notified of an urgent need, and order necessary medications, imaging studies or other supportive care interventions.
  • Communicate with primary treatment team regarding adverse reactions or urgent patient issues
  • Provide patient education.
  • Provide consultation to Outpatient Treatment Unit (OTU) nurses.
  • Ensure that the OTU has APP coverage at all times by working collaboratively with colleagues and covering for colleagues as needed when peers are on leave.
  • Provide urgent medical care for patients requiring same day evaluation
  • Provide medical care independently and/or in conjunction with attending physicians. Work in conjunction with MDs to provide continued clinical care in the outpatient setting
  • Review labs and imaging as appropriate
  • Assure appropriate setting of care (ambulatory, inpatient, critical care, acute care)
  • Document subjective and objective findings, assessment and plan clearly and accurately in progress notes according to OHSU guidelines
  • Provide comprehensive symptom management of complex patients by working collaboratively with oncology nurses, palliative care providers, social workers and others based on patient need
  • Maintain communication with primary care providers and other individuals and agencies (e.g. skilled nursing facilities) as appropriate
  • Deliver care to patients receiving treatment on clinical trials per protocol guidelines with timely notification of principal investigator/research nurse or study coordinator and appropriate documentation of adverse events

Quality And Research

  • Identify and engage in opportunities to improve the quality and efficiency of care provided to CHH patients
  • Promote use of appropriate research findings in practice to exceed the standards of care
  • Work with the physician leadership, nurse managers, nursing staff and social workers to prevent and/or respond to patient complaints

Nurse Practitioner

Required Qualifications

  • Current and active Nurse Practitioner license for the state of Oregon
  • State of Oregon Nurse Practitioner Prescriptive Privileges
  • DEA certificate

Physician Assistant

  • Current and active Physician Assistant license for the State of Oregon
  • DEA certificate

ACLS/BLS Certification Required

Preferred Qualifications

2+ years of APP experience in Hematology & Medical Oncology
