Position Description

A healthcare services company throughout New York is currently seeking a licensed Nurse Practitioner (NP) and/or Physician Assistant (PA) to join their Home Care team on a Part-time basis.

About the Opportunity:

  • Start Date: ASAP
  • Schedule: Part-time
  • Hours: Minimum of 9 per week

Responsibilities:

  • Review and make recommendations on consumer assessments for long term care services and supports
  • Maintain current knowledge of regulatory changes pertaining to quality and validity of assessments
  • Assess cases presented and utilizes clinical competencies to review and validate assessment outcomes
  • Work collaboratively to monitor and address clinical quality and gaps in care
  • Provide clinical and educational assistance to support project initiatives
  • Demonstrate communication skills necessary to interpret age-specific responses
  • Participate in performance, improvement, and quality assurance activities and related meetings
  • Provide clinical and educational support to meet program goals
  • Perform other duties as may be assigned by Management

Qualifications:

  • 2+ years of relevant work experience
  • Master's Degree from a Nurse Practitioner or Physician Assistant Program
  • NYS Nurse Practitioner (NP) and/or Physician Assistant (PA) license
  • Board Certified Nurse Practitioner (NP) and/or Physician Assistant (PA)
  • Computer savvy

Desired Skills:

  • Board Certified in Internal Medicine, Adult Health, Community Health, and/or Family Practice
  • Bilingual (English and Cantonese, Mandarin, Bengali, Haitian-Creole, Russian, or Spanish)
  • Experience working with the Adult / Geriatric populations.
  • Home Care background
  • Experience working with individuals with Physical and/or Developmental Disabilities
  • Knowledge of Long-Term Care services & programs
