Hybrid Nurse Practitioner | Greater Sacramento, CA Region 

We are seeking a compassionate, experienced Nurse Practitioner to join an innovative, patient-centered clinical team serving the Greater Sacramento region.

This full-time hybrid role provides excellent work-life balance, a collaborative interdisciplinary environment, and the opportunity to meaningfully impact the lives of patients with complex medical needs.

Position Highlights

• Schedule: Monday – Friday

• Hybrid Model: ~3–4 days in-person patient care at clinics; ~1–2 days remote administrative time

• Work-Life Balance: No nights, weekends, or on-call

• Compensation: $145,000 – $155,000 annually (based on experience)

Service Area

• Regional travel within South Sacramento County and the Northern San Joaquin Valley

What You’ll Do

• Deliver comprehensive care and education to patients with chronic and complex conditions

• Develop individualized care plans and guide treatment in collaboration with an integrated team

• Serve as a liaison among patients, families, physicians, and other providers to ensure coordinated care

• Provide both in-person care and telehealth services in a supportive, relationship-based model

Qualifications

• Current California NP license in good standing

• DEA license (or eligible within 90 days)

• BLS/CPR certification

• Minimum 2 years of NP experience (Family Medicine, Internal Medicine, Primary Care, or Hospitalist strongly preferred)

• Reliable personal transportation with valid driver’s license/insurance

• Strong assessment and care coordination skills

• Comfortable using Microsoft Office and electronic health records
