Overview

The Nurse Practitioner has primary responsibility for providing clinical services to patients. This includes the assessment, diagnosis and treatment of developmental and behavioral disorders. The Nurse Practitioner also works closely with the RFK CERC Medical Director with Quality Assurance projects related to patient care. The Montefiore NP maintains standards of the practice for nursing as defined by the American Nurses Association Code of Ethics, Nursing Scope and Standards of Practice and Nursing's Social Policy Statement. The NP actualizes the vision, mission, values, balanced scorecard performance measures for Montefiore Medical Center. The NP participates in performance improvement/research activities; maintains professional practice standards and clinical expertise; and demonstrates leadership skills. The NP demonstrates core and unit specific competencies based on the patient population served. The Nurse Practitioner will assess and provide care to a selected group of patients in collaboration with physicians. The NP will be responsible for carrying out the plan of care for his/her patient panel. The NP contributes to the actualization of the philosophy and objectives of the specific Division/ Department. He/she practices within the scope of practice in care delivery. He/she participates in performance improvement and research activities, and maintains professional practice standards and clinical expertise.

Responsibilities

  • Providing clinical services to patients. This includes the assessment, diagnosis and treatment of developmental and behavioral disorders including a complete health history and physical exam * Provide clinical services during 8 1/2-day clinical sessions per week.
  • In collaboration with physicians and other health care providers, coordinates care of patients and evaluates plan of care and patient toward health goals and modifies plan of care. * Appropriate care is provided as per standard of care and per collaborative practice agreement * Plan of care is communicated to all members of the health care team.
  • Collaborates with patient and/or involved others and/or agencies to formulate a plan of care * Appropriate plan is agreed upon between NP and involved others. A therapeutic relationship is established with patient and/or involved others. Identifies patient and/or involved others in satisfaction/dissatisfaction with plan of care. If patient and/or involved others express dissatisfaction implements conflict resolution skills, and escalates issues with plan of care.
  • Communicates effectively with involved others and members of the health team. * Demonstrates respect for cultural diversity with patients and others in the medical team. * Establishes collaborative and collegial relationships with health care members. * Communicates plan of care to its assigned patients and involved others.
  • Orders, performs and interprets diagnostic procedures and tests with the scope of privileges * Appropriate orders and communications are made.
  • Develops and analyzes differential diagnoses based on clinical and laboratory data. * Appropriate analyses are made
  • Participates in a team approach to health care * Participates in a team approach to health care by acting as liaison between subspecialist and other members of the primary care team. * Communicates effectively with members of the health care team. * Maintains communication with the interdisciplinary team through frequent interactions and staff conferences
  • Prescribes medications and educates the patient and/or involved others about the drug regimen, interactions and side effects. * Zero error tolerance
  • Demonstrates critical thinking skills to problem-solve and initiate clinical decision making * Assesses patient clinical status and attempts satisfactory resolution of patient clinical needs
  • Demonstrates responsibility for maintaining competencies * Demonstrates responsibility for own practice and ongoing development, including participation in at least one continuing education program annually. * Supports goals and activities of the team within the patient care area and at the MMC. * Contributes to the growth and development of staff and students through participation in peer review, mentor relationships, formal and informal education. * Participates in educational experiences within the practice setting. * Participates in activities contributing to the growth and development of self, colleagues
  • Is responsible and accountable for own practice * Critical thinking /problem solving is incorporated into nursing practice. * Contributes to problem identification and problem solving in the work environment, participating in task forces and groups as necessary * Supports the mission and vision of Montefiore Medical Center and the standards for nursing practice
  • Provides a safe environment for patients/involved others. * Orients patient/involved others to environment, assuring comprehension of information presented. * Administers medication error free. * Responds appropriately in an urgent/emergent situation.
  • Demonstrates leadership skills in the coordination and delivery of patient care * Coordinates an interdisciplinary of care. * Assures 100% of patient panel have a documented plan of care. * Delegates care of the patient to team members consistent with the capabilities and responsibilities of caregiver
  • Participates in quality of care projects related to clinical care as directed by the Medical Director * Coordinates quality of care and quality of improvement projects in various clinical areas * Participates in clinic meetings related to quality of care

Requirements

  • 4-6 years Required
  • Experience in the provision of developmental and/or behavioral health services.
  • Demonstrated effectiveness in clinical service provision in a multi-purpose center for persons with disabilities which includes clinical services., training, and research activities
  • Demonstrated leadership in developmental disabilities, consumer/family empowerment, and community building that supports full inclusion of individuals with disabilities
  • Demonstrated knowledge of best practices related to disability issues for all age groups, and state service delivery systems for individuals with disabilities.
