NJ Palliative Care Solutions, in partnership with Clara Maass Medical Center, is seeking a compassionate and experienced Nurse Practitioner (NP) or Physician Assistant (PA) to join our dedicated palliative care team. This is a unique opportunity to be part of a growing program that provides specialized care, comfort, and support to patients and families facing serious illnesses.

Responsibilities:

• Provide comprehensive palliative care services, including patient assessments, symptom management, and care planning.
• Collaborate with physicians, nurses, social workers, and other members of the interdisciplinary team.
• Support patients and families through education, counseling, and shared decision-making.
• Document and communicate clinical information clearly and effectively.
• Participate in care conferences, family meetings, and hospital-based initiatives to improve quality of care.
  
  

Qualifications:

• Licensed Nurse Practitioner (NP) or Physician Assistant (PA) in the State of New Jersey.
• Experience in palliative care, hospice, internal medicine, or geriatrics strongly preferred.
• Strong clinical assessment and communication skills.
• Ability to work collaboratively in a hospital-based setting.
