Stayton, Oregon - 4 days onsite, 1 day WFH admin day / potential for any virtual DR visits

$130 - 140k + Bonuses

Our client is an independently owned primary care clinic serving individuals and families in Stayton, Oregon. They focus on building strong relationships, preventive care, and treatment plans that empower patients to take charge of their health.

They are seeking a dedicated Family Nurse Practitioner or Physician Associate to join our growing team.

Position Overview

We are looking for a compassionate, detail-oriented Advanced Practice Provider who values long-term patient relationships and thrives in an independent practice. This role offers flexibility in scheduling and benefits.

Key Responsibilities Provide primary care for patients of all ages (pediatrics through geriatrics) Perform well-child visits, annual exams, chronic disease management, and acute visits Order and interpret diagnostic tests and labs Create and manage individualized treatment plans Prescribe and monitor medications Collaborate with clinic staff to coordinate care Support quality improvement and preventive health efforts Maintain accurate EHR documentation

Qualifications Licensed as a Family Nurse Practitioner or Physician Associate in Oregon (or eligible) National Board Certification (required) DEA license (or eligibility) Strong interpersonal and communication skills Passion for serving rural and underserved populations
