Nurse Practitioner – Home-Based Primary Care

Schedule: Monday–Friday, 8:00 AM – 5:00 PM ET

Travel: Within 60 minutes of home address (mileage reimbursed)

About the Role

Our client, a leading healthcare organization, is seeking a Nurse Practitioner (NP) to join their Home-Based Medical Care team. This role is ideal for clinicians who want to provide personalized, patient-centered care in a home setting while enjoying a predictable schedule and strong support system.

You’ll typically complete 5 visits per day, primarily in patients’ homes, with some telephonic visits. Documentation is completed during visits or from your home for telehealth appointments. Full training on the electronic medical record system will be provided.

Key Responsibilities

• Perform comprehensive assessments for new patients and ongoing care management.
• Develop and update individualized care plans in collaboration with the interdisciplinary team.
• Prescribe diagnostics and interventions to prevent unnecessary hospitalizations.
• Provide patient and caregiver education on disease management and wellness.
• Complete follow-up and post-discharge assessments by standard protocols.
• Participate in quality initiatives, including HEDIS measures.
• Perform basic clinical procedures (IV placement, blood draws, wound care, etc.).
• On-call for caseload during business hours; rotational after-hours on-call participation.

Qualifications

• Completion of an accredited Nurse Practitioner program.
• Current RN and NP license in Ohio.
• Minimum 2 years of clinical experience (primary care, home-based care, geriatrics, palliative, or urgent care preferred).
• Knowledge of CPT, ICD-10, HCPC codes, and clinical standards of care.
• Reliable transportation required.
