Position Description
Home-Based Medical Care Provider – Nurse Practitioner
Location:

About The Role
As a member of the Home-Based Medical Care (HBMC) interdisciplinary team, you’ll deliver direct, in-home medical care to patients within an assigned geography. You’ll collaborate closely with primary care providers (PCPs), HBMC teammates, consultants, and health plan medical directors to support patients’ goals of care throughout their healthcare journey.

About The Role
As a member of the Home-Based Medical Care (HBMC) interdisciplinary team, you’ll deliver direct, in-home medical care to patients within an assigned geography. You’ll collaborate closely with primary care providers (PCPs), HBMC teammates, consultants, and health plan medical directors to support patients’ goals of care throughout their healthcare journey.
Responsibilities
- Practice in accordance with state laws governing advanced practice nursing or physician assistant roles
- Build and maintain effective relationships with patients, caregivers, IDT members, PCPs, and facility staff
- Conduct initial comprehensive assessments for newly enrolled patients and provide ongoing care
- Develop and update patient management care plans based on condition changes or hospitalizations
- Review care plans with patients and legal representatives, involving PCPs or supervising physicians as needed
- Complete follow-up and post-discharge assessments per standard operating procedures
- Prescribe diagnostics and interventions to prevent unnecessary acute admissions
- Perform basic nursing procedures (e.g., IV placement, blood draws, injections, wound care)
- Consult with hospital, emergency, or post-acute teams following patient transfers
- Educate patients and caregivers on disease management and wellness promotion
- Provide support and education to staff as appropriate
- Participate in clinical meetings and quality improvement initiatives (e.g., HEDIS campaigns)
- Be on-call for caseload during business hours; participate in after-hours on-call rotation
- Stay current with medical research, technology, and best practices through continuing education
- Perform other duties as assigned
Education & Experience
- Completion of an accredited Nurse Practitioner program
- Minimum 2 years clinical experience in primary care, home-based care, geriatrics, palliative care, or emergency/urgent care preferred
- Familiarity with long-term care, community resources, and cost-effective care delivery
- Knowledge of CPT, ICD-10, HCPC codes, and clinical standards of care
- Awareness of NCQA, CMS, Medicaid/Medicare guidelines and benefit systems
- Current RN license (state-specific)
- Current NP license (state-specific)
- Reliable transportation required; must comply with Optum Motor Vehicle Safety policy if driving
