Pride Health is currently hiring a Nurse Practitioner for our Home Health client in Franklin County, OH! In this role, you will provide direct, in-home medical care to patients within a designated geographic area, with a focus on enhancing health outcomes and improving quality of life. You will collaborate closely with primary care providers, caregivers, and other healthcare professionals to deliver comprehensive, patient-centered care.

Shift: M-F 8a-5p

Pay: $64-$69 per hour

Contract Length: 3+ months/opportunity for extensions/permanent hire

Responsibilities:

• Perform comprehensive assessments and develop ongoing care plans.
• Provide follow-up care and interventions to reduce avoidable hospital admissions.
• Prescribe medications, order diagnostics, and manage treatment plans.
• Perform basic procedures such as IV placement, injections, wound care, and blood draws.
• Educate patients and families on disease management and wellness.
• Collaborate with interdisciplinary teams and participate in quality initiatives.

Requirements:

• Licensed NP or PA in OH
• 1+ year of clinical experience (home health or primary care preferred).
• Strong procedural skills (IV, wound care, injections, etc.).
• Excellent communication and care coordination skills.
