Summary of Essential Functions for the Position:

Committed to Caring, The Nurse Practitioner (NP) provides medical care to hospice patients of all ages with a focus on determining hospice eligibility and pain and symptom management including, but not limited to, history and physical exams; diagnosis and treatment of medical problems; education and counseling of individuals, families and groups. The Nurse Practitioner participates in the care of the hospice patient as a member of the interdisciplinary team. Under the supervision of the Hospice Medical Director and in collaboration with one or more physicians, the Nurse Practitioner may perform Face to Face patient visits to assist in determining hospice eligibility. The Nurse Practitioner may function in the role of Attending Physician only if selected by the patient or patient representative at the time of admission.

Qualifications

Qualifications / Licensure / Certification / Knowledge / Skills / Abilities / Shift / Coverage Area / Compensation:

• Currently licensed in the state(s) in which practicing.
• Two (2) years experience, Hospice preferred.
• CPR certified with American Heart Association or American Red Cross required.
• Proficiency with EMR systems (e.g., WellSky/Consolo).
• Ability to exercise initiative and independent judgment.
• Strong communication skills, both oral and in writing, with excellent interpersonal skills.
• Knowledge of Medicare Face to Face Encounter criteria.
• Knowledge of the interdisciplinary team concept.
• Ability to work collaboratively with referring, primary, consulting physicians and other members of the interdisciplinary team to provide appropriate palliative care to patients.
• Must demonstrate leadership, good customer relations skills and a commitment to providing quality service.
• Acceptance and ability to demonstrate support the core values and goals of Agency.
• Must have a current valid drivers license, auto liability insurance and reliable transportation.
• Must be on call at least once per month.
• 100% travel required.
  
  

Educational Requirements

• High School Diploma or GED. Graduate of an approved school of professional nursing.
  
  

Working Conditions & Physical Requirements

• Works indoors in Agency office and patient homes and travels to/from patient homes. Requires considerable physical effort most of the day including kneeling, squatting, reaching, twisting, climbing, walking, exposure to temperature and humidity changes and maximal assist in lifting and/or transferring of a 100 pound patient. Must possess sight/hearing senses or use appropriate adaptive devices that will enable senses to function at a level required to meet the essential duties of the position. Must provide evidence of annual TB test and other state-required tests or examinations. Be able to tolerate exposure to elements including, but not limited to, odors, blood, body fluids and excrements, adverse environmental conditions, and hazardous materials.
