Role Description

This is a part time role for a Family Nurse Practitioner specializing in wound care. The role is on-site and located in Woodland Hills Ca, Candidate will be required to travel as care is provided at patient's homes and/or via telemedicine visits. The Family Nurse Practitioner will be responsible for assessing and managing patients with acute and chronic wounds in collaboration with other healthcare professionals. Daily tasks include performing wound evaluations, developing and implementing treatment plans, and providing patient and family education related to wound care management. The Family Nurse Practitioner will also be responsible for documenting patient progress and outcomes, ensuring compliance with clinical protocols, and participating in interdisciplinary team meetings to coordinate patient care.

Qualifications

• Experience and minimum 2 years proven work experience skills in Nursing, Nurse Practitioners prior wound care
• Knowledge and expertise in Primary Care and Medicine
• Ability to manage wounds
• Excellent communication and interpersonal skills
• Ability to work collaboratively in a healthcare team
• Experience in wound care or related clinical practice
• Current CA state licensure and certification as a Family Nurse Practitioner
• Medicare credentialed
