In this role, you will work closely with advanced practice clinicians in a team-based care model. The Advanced Practice Provider will collaborate with interdisciplinary healthcare teams to deliver high-quality, patient-centered care. Within the advanced practice practitioner-physician relationship, this provider has the ability to exercise autonomy in medical decision making and provide a broad range of diagnostic and therapeutic services. May include responsibility for administrative duties and includes additional duties and parameters of specific individual contract and/or continuing education requirements.

Essential Functions

• Conduct physical exams, assess health status, order and interpret tests, prescribe medications, diagnose and treat illnesses.
• Monitor therapies and provide continuity of care.
• Counsel patient/family on preventive health care.

Education / Experience / Licenses / Certifications

Required

• Master’s degree and successful completion of accredited Program (Nurse Practitioner) program.
• National certification as a Nurse Practitioner, State NP license, and recertification required.
• CPR certification
