Seeking a Community Based Nurse Practitioner to join a thriving healthcare organization that is expanding into Northwest Chicago to provide primary care and support services to underserved populations.

General Job Responsibilities:

• Provide community-based medical and care-coordination services for recent hospital discharges.
• Manage a small panel of homebound members who require primary care.
• Resolve acute issue in the home and clinic or by directing member to most effective and efficient solutions.

Minimum requirements:

• Primary care or urgent care background needed
• 2+ years experience minimum as a NP
• Experience within community-based, home health, travel roles
• FQHC experience preferred
