With only 7 patient visits per day, a small patient panel, and a minimum of 30-minute visits, you'll have the time and space to provide the compassionate care your patients deserve.

Responsibilities

Responsibilities

• Deliver comprehensive primary-care to homebound, dual-eligible patients in their home
• Work collaboratively with physicians, nurse practitioners, nurses, therapists, social workers, and caregivers to deliver comprehensive and coordinated care to our patients in their home-conduct patient home visits
• Develop and implement comprehensive care plans
  
  

Qualifications

Qualifications

• Active Nurse Practitioner state license
• Master’s degree from accredited nursing school
• Valid drivers’ licenses
• Active DEA license
• Min 2 years of active Nurse Practitioner experience
