Central Health Management, Inc. is expanding our Kentucky practice and looking to contract with a qualified Nurse Practitioner with experience in primary care, wound care and/or long-term care. This role can be weekend only (3 weekends per month) or Monday - Friday based upon your availability.

In this role, you will provide patient's with either primary or wound care based upon the needs of the patient. In some cases, you will work with patients to transition from acute to home or acute to a longer term care situation. You will work in conjunction with other clinicians to ensure patient safety and care are the highest priorities.

To qualify for this role, you will have:

• MSN or DNP from an accredited Nurse Practitioner curriculum.
• Active and unencumbered license to practice in Kentucky or obtain.
• Active and unencumbered DEA license or obtain.
• Active BLS Certification.
• Wound Care certification or be willing to obtain.
• Ability to travel within a 50 mile radius.
• Excellent bedside manner and collaboration skills.
