Family Nurse Practitioner / Physician Assistant

Alpharetta, GA

Are you interested in treating a wide range of conditions, ranging from acute illnesses to chronic disease, often with significant independence in diagnostics and treatment planning?

Do you want to work with a leadership team with long tenure that fosters a stable work environment? Most of our leaders, physicians, APPs and nurses have been with the company for over 15 years.

We are seeking an APP to join our team in Forsyth County.

Here are the duties you will perform:

Primary Care

  • Manage chronic conditions like diabetes, hypertension, and asthma
  • Provide routine physical exams and preventive screenings
  • Administer immunizations and wellness checks

Acute and Urgent Care

  • Treat minor injuries such as sprains, lacerations, and fractures
  • Diagnose and manage common infections (e.g., respiratory, skin, urinary)
  • Triage medical emergencies and stabilize patients prior to hospital transfer if necessary

Mental Health Support

  • Screen for conditions like depression, anxiety, and PTSD
  • Provide ongoing medication management for stable psychiatric conditions, often in collaboration with mental health professionals

Substance Use Management

  • Assess and develop care plans for individuals with substance use disorders
  • Administer or coordinate Medication-Assisted Treatment (MAT), including Suboxone or Methadone, where available

Women’s Health

  • Perform Pap smears, pelvic exams, and STI testing and treatment
  • Offer prenatal and postnatal care to incarcerated pregnant women

Infectious Disease Management

  • Diagnose and treat conditions like tuberculosis, HIV, hepatitis, and STIs
  • Coordinate with public health agencies during outbreaks or for communicable disease control

Intake Assessments

  • Conduct comprehensive health screenings when individuals enter the facility
  • Document medical history, current medications, and potential health risks

Chronic Care Clinics

  • Provide ongoing follow-up appointments for patients with long-term health conditions
  • Adjust treatment plans based on patient progress and clinical guidelines

Health Education and Counseling

  • Educate patients on disease prevention, medication adherence, and self-care
  • Promote healthy lifestyle habits adapted for the correctional environment
