Position Description
Homebase Medical is getting back to basics. Our mission is to optimize the quality of life for homebound older adults, allowing them to live independently and with dignity in their own homes. We make house calls – enabling access to quality wrap-around services for homebound older adults with complex health needs while improving the quality of their lives, right at home.
We offer an array of medical and chronic care services and we’re backed by SCAN, a not-for-profit organization that has a history of supporting and improving the health and independence of older adults for over 45 years.
Homebase Medical is expanding rapidly into the King County - Kent, Federal Way and Auburn. We are currently searching for an experienced Palliative Care Nurse Practitioner. You will be responsible for palliative care in the home setting.
Key Duties
Tuberculosis Screening Policy

Key Duties
- Conduct Palliative Care Visits
- Comprehensive Assessment skills including pain management, symptom management, patient and family education.
- Focus on goals of care.
- Work collaboratively with our interprofessional palliative care team
- Support after hours, holiday and weekend on-call
- Support provider relationships with our clients
- Actively support the achievement of Homebase Medical’s Vision and Goals.
- We seek Rebels who are curious about AI and its power to transform how we operate and serve our members.
- Active Nurse Practitioner license in the State of Washington
- Board certification (Nurse Practitioner)
- DEA licensure
- 2 years or more of palliative care experience required
- CAPC preferred but not required
- Experience diagnosing and caring for Dementia/Alzheimer's patients.
- PayScale information: $65.00 - $90.00 per hour
