Leeds, Maine: We invite an experienced Family Nurse Practitioner or Physician Assistant to join a respected Federally Qualified Health Center serving a multicultural rural population of more than 10,000 in central Maine. Founded in the 1970s, this nonprofit operates three clinics between Lewiston (Maine’s second-largest city) and Augusta (the state capital), each about an hour from Portland. Care is delivered in a nationally recognized Patient-Centered Medical Home model, with integrated behavioral health, care managers, and patient assistance coordinators supporting a collaborative clinical team.

We seek a confident provider to deliver outpatient primary care services to patients of all ages. Full-time means a convenient four-day work week (40 hours), with part-time options possible. Weekend coverage is rotated across all providers. Practice alongside dedicated physicians and APPs, with opportunities to develop interests such as addiction medicine, supported through certification.
