Position Description

Eastchester Family Services is a nationally accredited, HRSA NHSC-approved, community-

based organization with offices in Atlanta, Cartersville, Marietta and Douglasville. Our agency is designed

to meet the diverse needs of today’s families by effectively integrating behavioral health and

primary care services to improve clinical outcomes, access to health care, and overall quality of

life for our clients and their families.

We are seeking a compassionate and skilled Family Nurse Practitioner (FNP) to join our agency

operating under an integrated services model. The FNP will provide holistic, patient-centered

care that addresses both physical and behavioral health needs. This role involves collaboration

with a multidisciplinary team—including psychiatrists, therapists, care coordinators, and primary

care providers—to ensure comprehensive treatment and improved health outcomes for patients

across the lifespan.

*This is an in-person, employee role. Candidates must reside in Georgia and be able to

commute to the office or designated meeting place for clients. Please expect that all services

will be provided in-person. The person selected for this role will work closely (on-site) with

Eastchester's community partners in Cobb, Fulton and Douglas counties.

Responsibilities

• Conduct comprehensive health assessments, including medical and psychiatric histories,

physical examinations, and diagnostic evaluations.

• Provide primary care services in coordination with behavioral health treatment plans.

• Prescribe medications, including psychotropic and medical management, in accordance

with scope of practice and applicable regulations.

• Develop, implement, and monitor individualized care plans addressing both physical and

mental health needs.

• Collaborate with behavioral health providers to ensure integrated care delivery and

continuity of treatment.

• Educate patients and families on diagnoses, treatment plans, preventive care, and lifestyle

modifications.

• Participate in case reviews, interdisciplinary team meetings, and quality improvement

initiatives.

• Document all patient interactions, diagnoses, and treatments in the EHR in compliance

with clinic policies, state, and federal guidelines.

• Provide crisis intervention and support when necessary.

• Stay current with best practices, evidence-based care, and continuing education relevant

to family practice and behavioral health integration.

Qualifications

• Current and unrestricted Family Nurse Practitioner (FNP) license in Georgia.

• National Board Certification as a Family Nurse Practitioner.

• Master’s or Doctoral degree in Nursing from an accredited program.

• DEA registration with authorization to prescribe.

• Experience in primary care, behavioral health, or an integrated care setting preferred.• Strong knowledge of psychopharmacology, chronic disease management, and behavioral

health conditions.

• Minimum of 2–3 years of clinical experience in primary care, behavioral health, or

integrated care preferred.

• Exceptional communication, collaboration, and patient engagement skills.

• Demonstrated success working with underserved, transient populations with clinical co-

morbidities (co-occurring mental health, substance abuse, and physical health problems).

• Commitment to delivering holistic, culturally competent, trauma-informed care.

• Experience using electronic health or medical records for patient notes (EHR | EMR).
