Position Description

We are seeking a compassionate and autonomous PRN Nurse Practitioner (NP) to join our growing healthcare team. This flexible, per-visit role focuses on providing high-quality, patient-centered primary care primarily through in-home visits in the Ogden area (approximately 90% of visits), with occasional telehealth appointments.

Unlike traditional clinic settings that emphasize patient volume, our model prioritizes quality, allowing you to spend the necessary time with each patient to deliver meaningful, personalized care. Key Responsibilities

• Conduct comprehensive primary care assessments during in-home and telehealth visits, including health history, physical exams, and diagnostic testing when necessary

• Diagnose and manage acute and chronic primary care conditions in both virtual and in-person settings

• Develop and manage personalized care plans based on patient needs and goals

• Prescribe and manage medications in accordance with Utah state licensure and scope of practice

• Educate and counsel patients and caregivers on preventive care, disease management, and lifestyle changes

• Coordinate care with other providers to ensure continuity and quality of care

• Accurately document all patient encounters in the electronic health record (EHR)

• Adhere to HIPAA and all applicable regulatory and professional standards

• Respond to both urgent and non-urgent patient needs during visits and follow-ups

Qualifications

• Must have a current Family Nurse Practitioner (FNP) certification and be able to see patients of all ages

• Active Nurse Practitioner license with prescriptive authority in Utah

• Minimum of 2 years of experience in primary care, family practice, or internal medicine

• Strong clinical judgment and ability to manage a broad range of conditions independently

• Proficiency with EHR systems and telehealth platforms

• Reliable transportation and valid driver’s license (required for house calls)

• Current CPR/BLS certification

• Excellent communication and patient engagement skills

Preferred Qualifications

• Bilingual (Spanish or other languages) a plus

• Experience with telemedicine or remote monitoring tools
