Position Description

Title: Nurse Practitioner

Location: Everett, WA

Duration: 4-6 Months approx.

Schedule:8-5PM, 40 hours

Hourly Blended Pay Rate: $65.00 – $70.00

Experience Required

Minimum 1+ year of clinical experience (preferably in long-term care or geriatrics)

Experience treating patients with complex medical, behavioral, and functional needs

Background in geriatric care, chronic disease management, and advanced illness care

Familiarity with end-of-life care planning is highly preferred

Certifications Required

Active, unrestricted NP or PA license in state of residence (or ability to obtain by start date)

NP Candidates:

Graduation from accredited MSN or DNP program

Board certified by AANP or ANCC in one of the following:

Family Nurse Practitioner (FNP)

Adult NP

Gerontology NP

Adult-Gerontology Acute Care NP

PA Candidates:

Graduate of accredited PA program

Certified by NCCPA (National Commission on Certification of Physician Assistants)

Active DEA license and prescriptive authority

Must be able to:

Lift 30 lbs, navigate stairs, perform physical assessments

Travel during shift across facilities

Full COVID-19 vaccination required

Ability to gain collaborative practice agreement (if required by state law)

Key Skills

Conducting comprehensive clinical assessments in senior living settings

Managing acute and chronic medical & behavioral conditions

Proficient in ICD-10 documentation and EMR systems

Strong interdisciplinary team collaboration

Skilled in advanced illness discussions and care planning

Excellent patient education and counseling abilities

Organized, adaptable, and able to manage autonomous patient care

Reliable transportation for field-based travel

Roles & Responsibilities

Provide integrated care to high-risk members in senior living facilities

Perform in-depth assessments and manage patients longitudinally

Lead advanced care planning conversations with patients and families

Collaborate with an interdisciplinary team to optimize care outcomes

Maintain accurate documentation for coding, compliance, and reporting

Ensure quality, compassionate care aligned with the ISNP program goals

Travel within assigned regions: Skagit, Whatcom, and Northern Snohomish Counties
