The Judge Group is seeking an experienced Nurse Practitioner to perform in-person, annual preventative health risk assessments for Medicare, Medicaid, and ACA members. Assessments are conducted in patients' homes across Richmond County, NY. This role is ideal for compassionate and motivated providers comfortable in residential environments and committed to high-quality care.

Schedule & Compensation

• Hours: day and evening appointments available (60–75 minutes per visit)
• Minimum Weekly Commitment: 30 appointments
• Compensation: competitive rate per completed assessment
• Mileage Reimbursement: after first 30 miles/day at IRS standard rate
• Location: Richmond County

Responsibilities

• Conduct comprehensive risk assessments based on health plan standards
• Document clinical findings and submit notes within 24 hours
• Respond to charting queries within 48 hours
• Provide minimum of 30 appointment slots per week

Required Skills & Competencies

• Knowledge of CMS and ACA risk adjustment models

Qualifications

• Master’s or Doctorate degree from accredited program (FNP, AGNP, ANP)
• Home health experience required within the last 5 years
• Minimum 3,600 hours of relevant medical experience

• Current NP license in New York
• Board Certification (ANCC, AANP, or NCCPA)
• Current Medicaid ID and Medicare PTAN
• Active BLS Certification
• Valid driver’s license and proof of auto insurance
