**This position starts October 1st and will run through approximately January**

We are seeking experienced Nurse Practitioners (NPs) and Physician Assistants (PAs) to perform initial health assessments for Medicaid members in their homes. We welcome candidates seeking full-time work to get the job done quickly, or those with limited weekly availability who want to pick up extra income.

Pay:

• $100/hr!

Locations & Openings:

• Brooklyn, NY – 4 openings

What You’ll Do

• Conduct comprehensive histories and physical exams in members’ homes
• Evaluate social determinants of health and deliver health education
• Order diagnostics, develop treatment plans, adjust as needed, and manage medications (as applicable to your license)
• Educate members on disease prevention, medication use, lifestyle changes, and self-care
• Complete and submit visit documentation within 24 hours

Must-Have Qualifications

• Active NP or PA license (NY)
• Minimum 1 year of clinical experience
• Medicaid ID
• Ability to commit to completing minimum of 30 assessments within 1 month (1hr/assessment = minimum 30 hours/month)
