Pride Health is currently hiring a Part-Time Nurse Practitioner for our client in Spokane, WA! You will collaborate closely with primary care providers, caregivers, and other healthcare professionals to deliver comprehensive, patient-centered care.

Join the Senior Community Care (SCC) program, a dynamic and integrated care delivery model designed to provide coordinated care for high-risk members in a facility setting. As part of an interdisciplinary team, you'll play a pivotal role in supporting the health and well-being of those who need it most.

Shift: .5 FTE approx. 20 hrs per week, Mondays and Wednesdays full 8 hr days, and Fridays 4 hrs.

Pay: $65-$85 per hour

Contract Length: 4-6 months, opportunities for extensions

Responsibilities:

• Conduct comprehensive assessments
• Effectively manage acute and chronic medical and behavioral conditions
• Ensure accurate and complete documentation, including ICD-10 conditions
• Communicate and collaborate with the interdisciplinary care team
• Conduct advanced illness and advanced care planning conversations
• Provide patients and caregivers with counseling and education

Requirements:

• Active and unrestricted WA NP or PA license
• Current, active DEA licensure/prescriptive authority.
• Ability to complete the job's physical requirements, including lifting a 30-pound bag in and out of the car, navigating stairs and various dwelling conditions, and sitting, standing, and kneeling to perform a physical assessment.
• Fully vaccinated against COVID-19
• 1 year of Long-Term Care experience preferred, but not required.
• Understanding of Geriatrics and Chronic Illness.
• Experience in meeting the medical needs of patients with complex behavioral, social, and/or functional needs.
