Position: Advanced Practice Clinician (NP or PA)

Location: Santa Clara County, CA (priority in/near San Jose)

Schedule: Full-time, Mon–Fri, ~8 a.m.–5 p.m.

About the Role

Join our Care Anywhere (CAW) team delivering proactive, in-home medical and social support to seniors. You’ll provide 4–5 home visits per day, complete geriatric assessments, support ongoing care, and help prevent avoidable hospitalizations. This role blends autonomy in the field with collaboration across an interdisciplinary team.

Responsibilities

• Conduct in-home assessments, screenings, and medication reviews
• Provide follow-up visits and continuity of care
• Educate patients/caregivers on disease management and prevention
• Coordinate with PCPs, specialists, and internal teams
• Complete timely EMR documentation
• Travel within ~25-mile radius; mileage reimbursed

Qualifications

• Master’s degree and active CA NP or PA license
• NPs: CA RN license, Furnishing number, NP board certification
• PAs: Active board certification
• NPI, DEA, BLS, valid CA driver’s license & auto insurance
• 1+ year of geriatric, primary care, or home-based care experience preferred

Compensation & Benefits

• Base pay: $130k–$195k
• Annual bonus up to 7.5% + RSU grant up to 15% of salary

• Mileage reimbursement at IRS rate
