Now Hiring: Fee-Based Nurse Practitioner – Fulton County, GA

Are you a compassionate and driven NP or PA looking to make a meaningful impact in your community? Join our team as a Fee-Based Initial Health Assessment (IHA) Provider and help improve health outcomes for Medicare, Medicaid, and ACA members through in-home and occasional virtual assessments.

Position Overview:

• Location: Fulton County, GA (must be willing to travel up to 50 minutes from home)
• Hours: Minimum of 30 appointment slots/month; flexible scheduling including days, evenings, and weekends
• Compensation: Fee-based per completed assessment + mileage reimbursement (IRS rate after first 30 miles/day) $95 per visit

Responsibilities:

• Conduct comprehensive health assessments in members’ homes
• Identify acute/chronic conditions and medication adherence
• Submit documentation within 24 hours via EMR
• Maintain compliance with all clinical, ethical, and regulatory standards
• Collaborate with clinical leadership and maintain a physician agreement

Requirements:

• Master’s or Doctorate from an accredited NP or PA program
• Active RN and NP or PA license in Georgia
• Board certification (ANCC, AANP, or NCCPA)
• Current Medicare PTAN, Medicaid ID, BLS certification
• Valid driver’s license and auto insurance
• Ability to pass background checks and drug screening

Preferred Qualifications:

• Experience in home health or in-home assessments
• Familiarity with CMS and ACA risk adjustment models
