Central Health Management, Inc. is seeking a board certified Nurse Practitioner to provide weekend coverage for 2 or 3 weekends per month (Friday, Sat, Sun) in a transitional care and/or post-acute care center in the Lexington, KY area.

This role will round on newly admitted and existing patients to provide direct patient care per the established treatment plan, evaluating the patients for any changes and adjusting the plan as medically appropriate. The role will work collaboratively with the internal medical staff and update charting timely to ensure the team is aware of any condition changes.

To qualify for this role, you will have:

• MSN or DNP from an accredited Nurse Practitioner curriculum.
• Active and unencumbered license to practice in Kentucky as a Nurse Practitioner.
• Active and unencumbered DEA license to prescribe in Kentucky.
• Active BLS and/or ACLS certification.
• Ability to work 2 or 3 weekends per month (Fri, Sat, Sun).
• Excellent communication and collaborative skills.
