At our Greensboro Hub, I provide comprehensive primary care with a focus on chronic disease management, behavioral health, preventive care, and transitions of care. I work closely with an interdisciplinary team to build personalized care plans and support members across their health journey.

• Chronic & acute care management
• Behavioral health (incl. SUD, depression, anxiety)
• Preventive care & wellness
• Collaborative, team-based care
• No weekends or on-call | M–F, 8am–5pm on-site

Location: 1439 E Cone Blvd, Greensboro, NC

Experience:

• NP or PA license to practice in the state of North Carolina
• 3+ years in primary care preferred, seeing all ages
