Position Description

Nurse Practitioner – Home-Based Primary Care (Part-Time, Flexible)

Location: Tuscaloosa, AL (within a 1-hour driving radius)

Schedule: 10 hours per week, Monday–Friday (flexible—work a couple hours per day or in larger blocks)

Travel: Mileage reimbursed

About the Role

Our client, a mission-driven healthcare organization, is seeking a part-time Nurse Practitioner (NP) to join their Home-Based Medical Care interdisciplinary team. This role is perfect for an NP who values flexibility, autonomy, and meaningful patient relationships—delivering care in members’ homes and via telephonic visits as appropriate.

You’ll document either in the member’s home during visits or from your home for telephonic encounters. Full training on the electronic medical record (EMR) will be provided.

What You’ll Do

  • Perform initial comprehensive assessments and provide ongoing care for assigned members.
  • Develop, implement, and update individualized care plans—coordinating with PCPs, HBMC teammates, consultants, and other health professionals.
  • Complete follow-up and post-discharge assessments by standard protocols.
  • Prescribe appropriate diagnostics and interventions to help prevent avoidable hospitalizations.
  • Educate patients and caregivers on disease processes, self-management, and wellness.
  • Perform basic procedures as needed (e.g., IV placement, blood draws, injections, Foley catheters, wound care, nasal packing).
  • Participate in quality initiatives (e.g., HEDIS measure campaigns) and clinical excellence meetings.
  • On-call for your caseload during business hours; may participate in a rotational after-hours on-call program as needed.
  • Maintain current knowledge via CE/CME and professional development.

Qualifications

  • Completion of an accredited Nurse Practitioner program.
  • Active RN and NP licenses in Alabama.
  • 2+ years of clinical experience preferred (primary care, home-based care, geriatrics, palliative care, or urgent/emergency care).
  • Knowledge of CPT, ICD-10, and HCPCS coding and clinical standards of care.
  • Awareness of NCQA, CMS guidelines, and Medicare/Medicaid helpful.
  • Reliable transportation and ability to travel within the assigned radius.
  • Comfort with basic nursing procedures (training and annual skills check provided).
